CONSENT, WAIVER, RELEASE

(For Adult Participants, 18 and older)

As an in-person attendee of this conference, there is a possibility you will be exposed to communicable diseases, including but not limited to COVID-19.

We ask all attendees to wear a face covering whenever you are within 6-feet of any other person in order to protect yourself and others. We strongly recommend additional safety precautions, including maintaining 6-foot social distancing and washing your hands frequently for at least 20 seconds.

Event/Adult Participant.  I am over the age of eighteen (18) and competent to sign this document.  I desire to participate in the following event, hereinafter “event”, which will be held at the Utah Valley Convention Center and other locations on the 25-26th day(s) of August 2020.  I understand that the event is being held, organized and sponsored by the following individuals and entities Governor's Office of Outdoor Recreation, Utah Media Group and various sponsors.  

Fitness and Inherent Risks.  I represent that I am physically fit and have been sufficiently trained to participate in the event.  I understand and acknowledge that this event is an extreme test of my physical and mental limits and carries with it the potential for death, serious injury, and property loss.  The foregoing risks include, but are not limited to, those caused by or associated with facilities, temperature, condition of athletes, equipment, travel, and the actions of others, including participants, volunteers, coaches, event officials, event monitors, and event producers/sponsors.  

Consent to Capture and Use Images.  I understand and agree that I may be photographed, filmed, and/or videotaped during this event, and I hereby consent that any photo, video, or film likeness, captured by the following event holders, producers, sponsors, organizers, and assigns, may be used by them for any legitimate purpose.

Waiver, Release of Liability, Hold Harmless.  In consideration my participation in the event, I hereby waive any claims that may arise out of this event, including, but not limited to, claims or actions related to my death, disability, personal injury, and property damage, and I hereby release and hold harmless the following entities and persons from all liability or claims that may arise from this event: Governor's Office of Outdoor Recreation, Utah Media Group, and various sponsors, together with their directors, officers, employees, volunteers, representatives, and agents. 

Application and Acknowledgment.  I acknowledge that this Consent, Waiver, and Release will be used by the event holders, sponsors, and organizers of the event.
